Transition Programme
At Activate we offer a bespoke transition programme tailored to the individual needs of each learner. Prior to joining Activate students will be offered the opportunity to visit and engage in taster sessions enabling them to become familiar with staff, peers and the site. During this time Student Services and Activates Transition team will diligently gather detailed information about each student to underpin their specialised curriculum and ensure student needs will be met from day one.
Each year throughout their course students and guardians are invited to Information Advice and Guidance meetings along with Annual Reviews. These are used to help shape future transitions and destination pathways. Activate works to ensure a person centred approach is used consistently.
Towards the end of their course students engage in preparations for transitioning out. Students and families are supported by the Transition Officer and Transition Job Coach to explore options for a meaningful, positive future destination.
Activate recognise transition can be a difficult process for many of our students and that a smooth process is essential in ensuring successful future placements.

Preparation for Adulthood
Activate aim to promote independence and work towards personal aspirations through consideration and use of the supportive tool created by the Department of Education. This tool identifies Preparation for Adulthood Outcomes across four main areas –

- Independent Living
- Community Inclusion
- Health
